03/25/2019 - From Pallium Canada: Dear LEAP facilitators and coordinators,

A dosing error has been found in the 2nd Edition of the Pallium Palliative Pocketbook. The error is located in Chapter 12, page 19 (i.e. 12-19) under Escitalopram (Cipralex®). The final bullet point outlined below is an error and should be crossed out:
•    Starting dose is usually 37.5mg OD (may be increased after 7 days to 75mg). Require at least 150mg per day for therapeutic effect. Usual dose is 150mg-225mg per day. Maximum dose is 375mg/day.
