How to Register for Victoria Hospice Care
Victoria Hospice works in partnership with Home & Community Care Nurses. These nurses are responsible for initial registration of our patients and ongoing coordination of skilled end-of-life care at home.
You are eligible to register* with Victoria Hospice services if:
- you live in an area of Greater Victoria served by Victoria Hospice
- the intent of your care is palliative (i.e. primarily comfort-oriented and not involving life-prolonging procedures)
- your family physician agrees that Victoria Hospice services are appropriate and desirable
- you are willing to accept added care in your home if necessary from community support systems including Home & Community Care Nursing and Community Health Care Workers
- you are currently residing at home or in an assisted-living complex
Note: * you may register before actually requiring any Victoria Hospice services
If you do not have a Home & Community Care Nurse at this time, you can ask your family physician to request a Home & Community Care Nursing referral for Victoria Hospice registration.
Your Home & Community Care Nurse will arrange a visit with you at home to provide more detailed information about the services that Victoria Hospice offers. After registering with Victoria Hospice, you can access our services by requesting them – often through your Home & Community Care Nurse.
